We operate more than 90 sites across San Diego County, including 29 primary care clinics, 23 behavioral health facilities, 10 physical rehabilitation clinics, nine dental clinics, five vision clinics, four outpatient substance use treatment programs, three mobile medical units, two mobile counseling centers, two urgent care centers, and a pharmacy. Our staff provides care to over 227,000 patients each year, of whom 91% are low-income and 29% are uninsured. FHCSD provides care to all. Services include, but are not limited to adult care, chronic disease management, pediatrics, comprehensive women’s care including obstetrics, dental, vision, case management, physical rehabilitation, speech therapy for children, vaccinations, infectious diseases, behavioral health, substance use counseling and a host of specialty services including cardiology, podiatry, endocrinology, dermatology, among others. FHCSD also offers supportive services to those who are unsheltered and in need of intensive case management. The breadth of our clinic locations, services and programs has grown over the last five decades, making us the largest community clinic provider of health care to the uninsured in the county and one of the top 10 largest community clinic organizations in the nation. We are also the largest health care safety-net provider, largest school-based health care provider and the largest mental health provider in the San Diego region. We are currently seeking a Nurse Practitioner or Physician Assistant to join our team Chula Vista Family Health Center. This location offers a wide range of care to our patients in the surrounding community and has a strong supportive team for our providers. Clinic Details Hours: Monday - Friday, 8:30am - 5:30pm ( close for lunch from 12:30pm – 1:30pm) Address: 251 Landis Avenue Chula Vista CA 91910 Location Website: https://www.fhcsd.org/chula-vista-family-health-center Duties and Responsibilities Provide quality and caring medical services to a diverse patient population Perform routine exams, common procedures and screenings Order and interpret laboratory tests, imaging, and point-of-care tests Evaluate patients with complex problems and refer to appropriate specialists Exercise cultural sensitivity and seek assistance with translations if needed Complete all required documentation in accordance with departmental standards Comply with electronic medical record requirements, including medication reconciliation, finalizing notes, clearing out personal pool, and participating in clinic pool Attend all mandatory meetings and minimize incident reports Use resources efficiently and practice medicine in a cost-effective manner Participate in Clinical Outcomes Program Maintain behavioral standards and respectful interactions with colleagues and patients Obtain good peer reviews, attend mandatory meetings and minimize incident reports Work toward meeting organizational targets for all metrics Participate in administrative duties and on-call schedule in a timely manner Must keep all required medical credentials active and in good standing Perform other duties as assigned Requirements: 2 years of experience in outpatient settings Current unrestricted license to practice in the State of California DEA license Current American Heart Association health care provider CPR (BLS), or Advanced Cardiac Life Support, Advanced Trauma Life Support, or Advance Resuscitation Training Experience with underserved, low-income, or homeless community members preferred Second language fluency desired, not required Compensation and Benefits $165,000 - $195,000 Full Time Annually (part time and per diem also available) CME Credit NHSC Loan Repay Eligibility Retirement Plan with 50% Employer Match Paid Time Off, Extended Sick Leave and Paid Holidays Medical/Dental/Vision/FSA/Life Insurance IndNurse We are excited to share that the salary range for this position is: $165,000.00 - $195,000.00 Information on our extensive benefits package can be found here: FHCSD Wellness – Employee Hub (gobenefits.net) FHCSD provides Equal Employment Opportunities to all employees and applicants for employment and prohibits discrimination and harassment of any type without regard to race, color, religion, age, sex, national origin, disability status, sexual orientation, gender identity or expression, or any other characteristic protected by federal, state, or local laws.
