Salary: $38.00 - $41.00 an hour Schedule: Monday - Friday 8 hour

Responsibilities Be the Best Palliative Licensed Practical Nurse

Graduated from an approved school of professional nursing One year
experience as a licensed practical nurse / Palliative experience
preferred, required in California Required Certifications and
Licensures Licensed to practice as a Licensed Practical Nurse in
the state of agency operation Possess and maintain valid CPR
certification while employed in a clinical role, preferred Must be
a licensed driver who can travel to all business locations Meet the
regulations and requirements of the state(s) in which program
provides services Responsibilities: As a Palliative Care LPN / LVN,
you will: Under the direction of an NP or physician, assist in
execution of the care plan, including routine and emergent visits
Contributes to the on-going care planning process, especially
regarding medical needs, with the care team and other agencies as
required Initiates and maintains communication with the NP,
interdisciplinary team, Palliative Director, and contracted medical
groups to coordinate optimal care and use of resources for the
patient/family Proves support for the Palliative patient and family
as new clinical needs are identified or questions arise, with the
referral to the clinician as appropriate Gathers information
regarding the patient for appropriateness for other service lines
that will best meet patient needs such as Hospice Our Investment in
