Job Description:
A company is looking for a Transitions of Care RN- 100%
Virtual.
Key Responsibilities
Conducts behavioral or clinical assessments to identify individual
member needs
Implements and coordinates health coaching plans and facilitates
referrals to health professionals
Monitors and evaluates the effectiveness of interventions and
modifies plans as needed
Required Qualifications
Associate's degree in nursing
Minimum of 2 years of condition-specific clinical or home
health/discharge planning experience
Current unrestricted RN license in applicable state(s)
Bilingual candidates are strongly preferred
Experience in care of adult and chronically ill patients is
preferred
