Job Description:
A company is looking for a Senior Care Manager (RN).
Key Responsibilities
Develops and assesses ongoing care plans, collaborating with
providers to address members' unmet needs
Coordinates care between members, families, and care provider teams
to ensure appropriate services
Monitors care plans and member status, revising plans as necessary
to meet goals and needs
Required Qualifications
Degree from an Accredited School of Nursing or a Bachelor's degree
in Nursing
4 - 6 years of related experience
RN - Registered Nurse - State Licensure and/or Compact State
Licensure required
Hospital and Case Management experience preferred; home health
experience and utilization management a plus
